Accessing your patient’s clinical information
Health Information Services are responsible for the collection, processing, storage, retrieval and dissemination of patient information, in both paper and scanned/ electronic format.
Northern Health uploads patient discharge summaries and pathology results to My Health Record. Before you contact the Health Information Services team please check the patient’s My Health Record. For more information on how to do this please click here.
How to request your patient’s Information:
GP- Nominated as GP on patient’s record with Northern Health
