Primary Care Liaison
The Primary Care Liaison department is the interface for general practitioners to contact Northern Health. The department works closely with the Primary Health Networks to provide education and projects that meet the needs of general practitioners to enhance patient care.
Primary Care Liaison provides advice and support to GPs and other community healthcare professionals in order to address communication gaps, support community care, efficient and quality clinical handover and cross health sector coordination and care. This includes information such as navigating services, compliments and complaints, follow-up of hospital care, clinical care issues and referrals.
The unit also supports a Shared Maternity Care model and provides Royal Australian College of General Practitioners (RACGP) endorsed education activities.
