What care and support is provided through COVID Monitoring?

All Victorians who test positive for COVID-19 are contacted by a triage and assessment (intake) provider. Where consent is provided, each case is triaged into one of three pathways (low, medium, or high severity) based on clinical risk, severity of illness and social needs. This ensures the clinical team can transfer people with COVID-19 to higher or lower care pathways, depending on their condition over the course of their self-isolation. The program aims to give all patients with COVID-19 access to high quality care and support at the right time and in the right setting.

Care will  be provided in the home through telehealth.

You will also receive access to the social and welfare supports you will need to isolate safely. The program can connect you with the right care to assist you to manage any other health or social needs you have.

If you are unable to isolate in your home, COVID-19 Positive Care Pathways program will organise an assessment for you to access emergency accommodation.

How we will help you

If you test positive for COVID-19 and live in the Northern Health catchment, we will help you to stay well and safe.  You can expect the following:

  • Phone Contact for an initial assessment
  • Daily Monitoring of your symptoms until you complete your mandatory isolation period (via electronic message to your mobile phone)
  • Phone support to help you manage your symptoms at home where possible
  • Information about how to get food and supplies while in isolation at home
  • Support to help you care for your children if your COVID symptoms worsen and you are admitted to hospital
  • Interpreter service, if you need them

What you need to do if you test positive to COVID-19

  • Isolate in your home to prevent the spread of the virus to other people
  • Complete a daily self-check survey to let us know how you are feeling
  • Let us know if you have any other symptoms
  • Let us know if you need additional support so that you can stay safe and well
