Primary Care Liaison (PCL)

Northern Health's Primary Care Liaison (PCL) service is part of the Northern Health Partnerships portfolio and provides assistance to Northern Health staff, the GP community and primary care agencies to develop links and opportunities for collaboration. The focus of the Unit is on improving communication pathways so that patient access to services is improved and outcomes enhanced.

For GP's

Primary Care Liaison provides advice and support to GPs and other community healthcare professionals in order to address communication gaps, support community care, efficient and quality clinical handover and cross health sector coordination and care. This includes information such as navigating services, compliments and complaints, follow-up of hospital care, clinical care issues and referrals.

Medicare Locals / Primary Care Partnerships

Primary Care Liaison works closely with Primary Health Networks and Primary Care Partnerships to:
• develop partnerships
• facilitate joint planning and care
• provide education
• address gaps in the service
• achieve better health and wellbeing outcomes for the community
• represent NH on Governance groups, Steering Committees and Focus groups
• plan for growth
• shape health care reform
• participate in health promotion
