What is the Service?

The Geriatric Evaluation Management at Residential Care program (GEM @ Resi ) provides care to people who are well enough to be cared for at an aged care facility but still require support from the NH GEM doctors, nurses and allied health. Northern Health’s GEM @ Resi beds are located at two local residential care facilities. Many people can benefit from this program but people who are unable to weight bear following a fracture of a bone, need additional time to finalise long term care arrangements or require modifications at home prior to their return are most frequently referred to GEM@Resi.

What can I expect from the service?

Whilst on the program, you and your family member will be supported by a multi-disciplinary team consisting of Geriatrician, physiotherapist, allied health assistant and some case management. Nursing and personal care are provided by the residential care facility staff.

How much does it cost?

There are no daily fees associated with the GEM @ Resi program however you or your family member will be responsible for any costs associated with medication during the duration of the program.

Service Overview

GEM@Resi provides an alternative care pathway for patients that no longer require a hospital admission but need time to finalise their long term care arrangements, optimise their functional capacity or their home situation. Patients are supported by a limited multi-disciplinary team and are provided with low intensity therapy (such as physiotherapy), case management and either nursing support and/or personal care.

Inclusion criteria:

  • Patients are medically/clinically stable.
  • Patients waiting for a residential care bed to become available
  • Patient who are unable to weight bear safely at home prior to returning to subacute for an in-patient period of rehabilitation or GEM
  • Patients waiting for carer availability (due to carer injury/health issues)
  • Patients waiting on structural/environmental issues to be resolved in their home environment prior to their return home.

Exclusion criteria:

  • Patients requiring acute or sub-acute in-patient care.
  • Patients requiring intervention from multiple allied health disciplines other than physiotherapy.
  • Patients not consenting to the program.
  • Aggressive/ intrusive wandering behaviours.
  • Patients isolated due to communicable infectious diseases.
  • Patients with care requirements that cannot be managed by an aged care facility (e.g. VAC therapy).

How to Refer:

  1. Clients are referred to GEM@Resi via the GEM@Resi referrals email address utilising the Generic Referral – GEM@Resi – 330280 form.

The Clinical Coordinator will assess the immediate needs and discuss a plan of care to determine if the program can provide an appropriate package.

  • The patients verbal consent,
  • ISBAR compliance
  • Referrals from within Northern Health require a long-term drug chart, goals of patient care and a medication management plan to be completed and sent with the patient.

Cost

No Daily fee – GEM @ Resi is an admitted service funded under GEM.
Patients will be responsible for covering the costs of their medication via the pharmacy associated with the facility.

Medicare ineligible patients will be discussed on a case by case basis.
