What is the Service?

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides assessment and advice about medical conditions related to ageing such as falls, incontinence, difficulty walking or depressed mood. 

What can I expect from the service?

You will be invited to attend a clinic. The doctor that you will see is a Geriatrician.

The Geriatrician will ask you questions about your health, will ask you to do some tasks and may examine you. He or she will work out a management plan with you which may include medication, therapy or other services. A copy of the findings and the plan of care will be sent to your GP.

The service runs across 3 Northern Health sites:

Broadmeadows Hospital

Clinic operates on: Thursday

Bundoora

Clinic operates on: Monday and Thursday

Craigieburn

Clinic operates on: Monday and Friday

What do I bring/need for my appointment?

Please bring a list of current medications, relevant scans or results.

 How much does it cost?

    • No cost to attend the Geriatric Evaluation and Management Clinic.
    • A fee of $5-10 may be incurred if you are referred on to therapy services.
  • If client is ineligible for Medicare the referral for the service will need to be reviewed by management.

Service/Clinic overview

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides a Geriatrician assessment, diagnosis, and management of complex medical conditions. The Geriatrician works closely with the client’s GP in order to coordinate the medical management of the client.

 What can be provided:

Geriatrician completing assessment, diagnosis, and management of complex medical conditions. The Geriatrician works closely with the client’s GP in order to coordinate the medical management of the client.

Clinic operates on: Monday and Friday

Inclusion criteria:

  • Client must be over 65 years
  • Requires medical consultation regarding client’s complex medical issues including medical management and/- or medication review
  • Requires further medical input where client already has a formal diagnosis affecting cognition e.g. Alzheimer’s, Dementia

 Exclusion criteria:

  • Client has seen or is seeing multiple specialists addressing current medical issues.
  • Referral for pain management where there are no other co-morbidities/complexity
  • Referral for capacity assessment

 How to Refer:

Referral must be from a GP or Consultant/Specialist. Referral must include:

  • Past medical history including Psychiatric history.
  • Current presenting issues
  • List of current medications.
  • Results of all relevant current pathology and radiology
