What is the Service?
Ward One is a 21-bed ward located at the Bundoora campus of Northern Health. Your medical team and the geriatrician have decided that you require ongoing care before returning to the community. This will have been discussed with you and your family. Ward One is a Geriatric Evaluation and Management Unit.
What can I expect from the service?
Soon after your arrival, you will be welcomed and assessed by the nursing, medical, and allied health staff. These assessments and your own input will be used to develop a planned approach to reach the goals needed for you to return to the community, use an interim care program in the short term or find residential care. We work as a team and believe that the teamwork includes you and your family.
What service can patients expect to receive?
We have a conference once a week where the clinical team discusses each patient and develops an overall plan with realistic goals, including a probable discharge date. Also at this meeting a Key Contact Person is appointed. This team member will make him or herself known to you and will be the person who will coordinate your care in hospital and see that your plans for discharge are progressing to completion.
The Team:
Our team is made up of nursing staff, medical staff, physiotherapists, occupational therapists, social workers, pharmacists, dieticians, speech pathologists and allied health assistants.
What do I bring/need for my appointment?
- 2-3 sets of day clothes, including a cardigan/jumper.
- Sensible footwear – flat, lace-up or slip on shoes with grip
- Night attire, sensible slippers
- Toiletries
- Safety net card
- Pension / DVA card
- Medicare card
- Current walking aid
