Residential In Reach (RIR)


Residential In-Reach is a rapid response service made up of Northern Health Doctors and Nurses specialised in Geriatrics (care of the older person).

Our Residential In-Reach team provides onsite and telehealth assessment, treatment and follow-up for people living in Residential Aged Care Facilities who are acutely unwell or have complex health conditions.

Our goal is to provide timely, appropriate care in the residential aged care setting and, where clinically appropriate, help avoid an unnecessary presentation to the Emergency Department.


What to expect from the service?

We provide timely, comprehensive care to support residents in the comfort of their aged care facility. Our services include:

  • Health assessment for acute, chronic and deteriorating conditions
  • Treatment and Administration of medication
  • Medication review
  • SPC and IDC care, including trial of void (TOV)
  • Iron infusions
  • Coordination of X-Rays, ECGs and pathology
  • Family support meetings
  • Education for residents, families and carers
  • Goals of patient care discussions
  • Palliative care support
  • Post hospital follow up
  • Support for facility to recognise and manage early clinical deterioration

Monday to Sunday 8.00am to 4.00pm

Non-urgent referrals:


Following receipt of referral, referrals are triaged and processed within 24 โ€“ 48 hours.

Processing involves checking eligibility, confirming referral information, and triaging the priority of the referral.
