Pulmonary rehabilitation, delivered at home

Virtual Pulmonary Rehabilitation is an evidence-based program that helps people living with chronic lung disease improve breathlessness, increase exercise capacity, build strength and confidence, and stay well. It delivers the same core benefits as traditional pulmonary rehabilitation, using a secure digital platform so you can take part from your own home.

What is pulmonary rehabilitation?

Pulmonary rehabilitation is a structured program that combines exercise, education and support. It is recommended for people living with long-term chronic lung conditions and has been shown to:

  • Reduce breathlessness
  • Improve fitness and daily function
  • Increase confidence and independence
  • Reduce flare-ups and hospital visits
  • Improve quality of life

Virtual Pulmonary Rehabilitation delivers this care in a flexible, accessible way, without the need to travel to hospital or health centres.

What does the program involve?

The program is tailored to your goals, symptoms and confidence level and runs for 8 weeks. It includes:

  • Individual assessment by the pulmonary rehabilitation team
  • Telehealth video consultations with physiotherapists, nurses, pharmacists, health psychologists and doctors, as clinically indicated
  • Personalised exercise program to complete at home
  • Educational resources to help you understand and manage your condition
  • Monitoring and support from the care team

After the eight-week program, you will be supported to continue exercising and managing your condition independently, with an optional four-week self-management program.

Who can take part?

This program is designed for people living with chronic lung conditions such as COPD or interstitial lung disease. Your healthcare team will confirm if the program is suitable for you.

How does virtual care work?

You will take part using My Health@Northern, a secure app for virtual care. The app allows you to attend virtual appointments, access program resources and stay connected with your care team. The team will guide you through the setup and provide support if needed.

Using the app, you will be able to:

  • Join video consultations from home
  • Communicate with your care team
  • Receive reminders and helpful educational resources
  • Track your exercise progress over time

Is the program safe?

Safety is a core part of the program. Before you start, the team will review your health needs and your home environment. Exercise programs are tailored to your ability, and clear support and escalation pathways are in place if your symptoms change.

How do I get referred?

Referrals are made by your:

  • GP
  • Respiratory physician
  • Hospital or community health service
  • Any healthcare professional
