Qualified and Expert Case Managers
Our case managers are all experienced health professionals with tertiary qualifications in nursing or allied health (e.g. social work and occupational therapy). We are experts within the community aged care industry – with the necessary knowledge and skills to assist with all your ongoing support issues and health needs. Our case managers can become your individual consultants and simplify the process of navigating complex aged and health care systems.
Consumer Directed Care
We are passionate about consumer-directed care. We will ensure that a personalised and flexible package of support services is designed with you; specifically based on your identified needs and expressed aspirations. Packages are flexible and can be adjusted as your needs, or your carers’ needs, change over time.
Your call will be personally responded to and your case manager will contact you regularly. We are a large service consisting of case managers (some of whom are bi-lingual), administrative staff, two dedicated clinical coordinators, a program manager, on-call case management systems and an after-hours support service. Someone will always be available to assist you.
Northern Health Connections
Our connection with Northern Health can assist you to access additional quality health services and interpreters of all nationalities and languages. Staff receive regular professional development training and are experienced in the use of interpreters.
Flexibility and Choice
Our many service contracts give you greater flexibility and choice. You are not restricted to using one agency. Instead, you can select from a number of quality agencies to ensure your specific needs and preferences are met.
Value for Money
We represent value for money. You can select your preferred level of case management involvement and budget to get the best value from your package. We do not charge a daily care fee and we will not deduct interpreter costs from your individual package funds.
