The Older Persons Care Hub (OPCH) delivers specialist geriatric care to older adults in their homes, focusing on those at risk of potentially avoidable hospital admissions. Our team recognises the value of home-based care and conducts comprehensive in-home assessments, providing short-term, targeted interventions to support safety and independence within the community.

Our team is staffed by various clinicians with a specialty in Geriatric care. The team includes:
- Physiotherapist
- Occupational Therapist
- Nursing
- Social Work
- Geriatrician
What to expect from the service?
Patients accepted by the OPCH will receive an initial phone call assessment within 24 hours of receiving the referral (where possible). This initial assessment enables a decision as to whether a home visit is required and who the most appropriate health discipline/s to provide this.
Our clinicians will visit you in your home and complete a comprehensive assessment. From this assessment, we will determine the most appropriate interventions to keep you safe at home. These interventions might include providing equipment to prevent you from falling or referring you to longer term services to support your ongoing care needs in the community.
You might be referred to our Geriatrician clinic, where our Geriatrician can investigate medical concerns related to ageing.
The OPCH is a rapid engagement service with the goal of discharging all patients within two weeks. Where longer-term support is required, our team will ensure that appropriate services are in place to provide ongoing care.
Prior to discharge, OPCH staff will contact you to confirm that you have received all relevant information regarding the assessments and interventions provided during your care.
