Northern Hospital Outreach Post suicidal Engagement (HOPE)


HOPE provides 12 weeks of intensive and individualised therapeutic care and support for people post suicidal ideation or attempt, where the driver behind the crisis is psychosocial stress. Hope is not a crisis program for people with active suicidal thinking.

The program offers outreach or in clinic care for people who reside in the Merri-bek, Hume, Darebin and Whittlesea municipalities between 18-65 years old. The delivery of care is in an integrated model that encompasses well-being support, peer & carer support & Clinical support

What to expect from the service?

  • 12 weeks of holistic, proactive support tailored to the individualโ€™s unique needs  
  • Development of an individualised safety plan  
  • Psychologically-focused sessions to increase coping skills  
  • Practical support and assistance to address drivers of suicidality  
  • Connection to community supports  
  • Family and carer support  
  • Alcohol and Other Drug brief intervention/support/linkage 
  • Peer Support  
  • Face-to-face, telehealth and telephone contact  
  • Sharing of appropriate resources  
  • Liaising with current community supports i.e. GP, psychologist, or other 

The referral process to Northern HOPE is through intake. Referrals are accepted from  

Emergency Departments (including Emergency Mental Health)  

Inpatient units   

Prevention and Recovery Centres (PARCs) 

Adult and Older Adult Community Mental Health Teams (including Acute Crisis Intervention Service)  

HOPE – Whittlesea & Darebin

Referrals for people who are experiencing any mental health crisis should be made by contacting triage. HOPE do not accept referrals from individuals or primary care providers.
