Geriatric Evaluation and Management


Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides assessment and advice about medical conditions related to ageing such as falls, incontinence, difficulty walking or depressed mood.

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides a Geriatrician assessment, diagnosis, and management of complex medical conditions. The Geriatrician works closely with the clientโ€™s GP in order to coordinate the medical management of the client.


What to expect from the service?

You will be invited to attend a clinic. The doctor that you will see is a Geriatrician. The Geriatrician will ask you questions about your health, will ask you to do some tasks and may examine you. He or she will work out a management plan with you which may include medication, therapy or other services. A copy of the findings and the plan of care will be sent to your GP.


Referral must be from a GP or Consultant/Specialist. Referral must include:

  • Past medical history including Psychiatric history.
  • Current presenting issues
  • List of current medications.
  • Results of all relevant current pathology and radiology
  • Client must be over 65 years
  • Requires medical consultation regarding clientโ€™s complex medical issues including medical management and/- or medication review
  • Requires further medical input where client already has a formal diagnosis affecting cognition e.g. Alzheimerโ€™s, Dementia
