Community Therapy Service (CTS)


The role of the Community Therapy Programs is to optimise a personโ€™s functional ability and independence

The Community Therapy Service โ€“ offers short term goal directed rehabilitation aimed at improving clientโ€™s function and independence following a change in function due to recent acute medical / health event. This is achieved by developing a care plan based on goals that are negotiated between the client and their team (see below for details of team members).

Patients may be seen in their own home, or in one of our centres, or via telehealth or a combination, dependent on their needs and goals. The patients may be seen by single disciplines or by multiple disciplines depending on need. Therapy can either be delivered in a group or individual setting. The service is provided at Bundoora, Broadmeadows Hospital and Craigieburn campuses.
Patients may participate in exercises or activities with the clinicians and they may recommend a program for patients to complete at home. The length of time a patient will be involved with CTS will be established with them, taking into account their goals and capabilities.


What to expect from the service?

The service we provide is individualised, time-limited (approximately 6-8 weeks), and goal-centred to improve health outcomes. Rehabilitation services are delivered according to a care plan negotiated with the client and their carers.

  • Orthotics
  • Speech Pathology
  • Psychology
  • Neuropsychology
  • Social Work
  • Dietetics

There may be a small cost for our service, which we will explain to you when we call to discuss your treatment plan.


Inclusion criteria

  • Reside in NH catchment and Recent (<8 weeks) surgery or hospitalisation with/and
  • Related rehabilitative goals for therapy which may be focused on function or cognition with/and
  • Must agree to actively participate in therapy with/and
  • Time limited intervention period averaging 6 โ€“ 10 weeks

Exclusion criteria

  • Client is eligible to receive the equivalent service through TAC, WorkCover or other compensation claim
  • Client has no goals
  • If client is ineligible for Medicare the referral for the service will need to be reviewed by management.

How to refer

Referral from any source.

Referral preferably will include:

  • Past medical history including Psychiatric history.
  • Current presenting issues
  • List of current medications.
  • Results of all relevant current pathology and radiology
  • Goals
