Northern Health is an approved provider for Commonwealth Home Support Program (CHSP) for Allied Health, Continence Nursing and Community Nursing. Northern Health CHSP is underpinned by a wellness approach which focuses on maximising a person’s independence. We aim to build your strengths, capacity and goals to remain independent and live safely at home.

What can I expect from the service?

Our team has been providing Allied Health and Continence Nursing Services to the community for over 25 years. Our team is based at Bundoora and Broadmeadows campus and can provide services either at your home or our centre.

We provide the following services:

  • Occupational Therapy
  • Physiotherapy
  • Speech Pathology
  • Dietetics
  • Podiatry
  • Orthotics
  • Exercise Physiology
  • Allied Health Assistants
  • Continence Nursing
  • Community Nursing

Our teams have specialist skills in working with older people with chronic and/or progressive neurological conditions as well as those with ageing-related needs.

Eligibility:

CHSP is available for those above 65 years old or 50 years for Aboriginal and/or Torres Strait Islander persons. You will require a referral from My Aged Care.
Depending on your needs we can provide single discipline or multidisciplinary interventions including:

  • Prescription of aids and equipment
  • Advice on home modifications
  • Therapy to improve or maintain physical function
  • Therapy to improve or maintain independence in daily activities
  • Carer education
  • Swallowing and communication
  • Dietary and nutritional advice and support
  • Treatment and prevention of conditions for lower limb and feet
  • Linking in with relevant services
  • Assessment and education re: Continence issues

What are my rights and responsibilities?
The new Aged Care Act 2024 (Cth) puts the rights of older people first. It includes a Statement of Rights for older people accessing aged care and a Code of Conduct for providers and people providing your care.

The Statement of Rights replaced the Charter of Aged Care Rights on 1 November 2025. The Statement explains what rights older people have when accessing aged care services funded by the Australian Government. The Code of Conduct sets out how providers and the people providing your care must behave and treat you and strengthens powers to protect you from services or workers that make you feel unsafe, taken advantage of, or disrespected. Service Overview

Northern Health is an approved provider for Commonwealth Home Support Program (CHSP).

We provide the following services under CHSP:

  • Allied Health (Occupational Therapy, Physiotherapy, Speech Pathology, Dietetics, Podiatry, Exercise Physiology, Social Work)
  • Continence Nursing
  • Community Nursing

Inclusion criteria:

CHSP is available to people over 65 years old, or 50 years for Aboriginal and/or Torres Strait Islander persons. Referral to CHSP is through My Aged Care.

Exclusion criteria:

Under Commonwealth Legislation, CHSP services cannot be offered to:

  • Permanent residents of residential aged care facilities, where a resident’s accommodation contract provides for similar services to those under CHSP.
  • Where needs can be met by other more appropriate Commonwealth funded programs such as a Home Care Package or Support at Home Classification
