Northern Health works collaboratively with its community partners and local health care providers to ensure patients receive the right treatment, in the right place, in a timely and appropriate manner. For information regarding the services provided at Northern Health, please follow the link for specific service information.

Referrals should be made through HealthLink eReferral Solution. Referrals are made through your clinicโ€™s MyHealthlink Portal.

HealthLinks SmartForms

Northern Health Specialist Clinics has now switched to HealthLink SmartForms for receiving referrals from GPs.

All paper-based referrals to Northern Health should be sent via fax:

Northern Health utilise the  Department of Healthโ€™s Statewide Criteria where available, in order to ensure equity of care for patients who require specialised care, assessment and management

State-wide referral criteria also contain minimum required information that allow a clinician to prioritise the need for an appointment and increase their ability to appropriately manage the clinical condition from a patients first appointment with a specialist.

If a referral does not contain the minimum required information, we will ask clinicians to re-submit the referral with the appropriate information.

Not all specialities or health conditions are covered by the state-wide criteria. In that case, Northern Health also utilise HealthPathways (see below)

HealthPathways Melbourne provides referring GPs with a comprehensive suite of clinical pathways, including guidance on when to refer and required information to accompany a referral.
