Geriatric Evaluation and Management

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides assessment and advice about medical conditions related to ageing such as falls, incontinence, difficulty walking or depressed mood.

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides a Geriatrician assessment, diagnosis, and management of complex medical conditions. The Geriatrician works closely with the client’s GP in order to coordinate the medical management of the client.

What to expect from the service?

You will be invited to attend a clinic. The doctor that you will see is a Geriatrician. The Geriatrician will ask you questions about your health, will ask you to do some tasks and may examine you. He or she will work out a management plan with you which may include medication, therapy or other services. A copy of the findings and the plan of care will be sent to your GP.

Referral must be from a GP or Consultant/Specialist. Referral must include:

Past medical history including Psychiatric history.
Current presenting issues
List of current medications.
Results of all relevant current pathology and radiology
Client must be over 65 years
Requires medical consultation regarding client’s complex medical issues including medical management and/- or medication review
Requires further medical input where client already has a formal diagnosis affecting cognition e.g. Alzheimer’s, Dementia
