Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides assessment and advice about medical conditions related to ageing such as falls, incontinence, difficulty walking or depressed mood.

Geriatric Evaluation and Management (GEM) Clinic is a medical outpatients clinic that provides a Geriatrician assessment, diagnosis, and management of complex medical conditions. The Geriatrician works closely with the clientโs GP in order to coordinate the medical management of the client.
What to expect from the service?
You will be invited to attend a clinic. The doctor that you will see is a Geriatrician. The Geriatrician will ask you questions about your health, will ask you to do some tasks and may examine you. He or she will work out a management plan with you which may include medication, therapy or other services. A copy of the findings and the plan of care will be sent to your GP.
