The Chronic Wound Service provides an assessment and treatment to patients in the community with chronic or complex wounds. Services are offered centre based and home based including Residential Aged Care Facilities.

The Chronic Wound Service provides multidisciplinary service to people who have wounds that are not healing as expected. The team provides:
- A comprehensive general and wound assessment including diagnosis
- Provide a plan for wound care
- Interventions to assist with wound healing that might include compression therapy, debridement and offloading
- Referral for additional investigations or input from other Northern Health specialist teams
- Providing education about wound healing, skin care and other factors impacting on wound healing
What to expect from the service?
Clients undergo a comprehensive assessment including diagnosis, treatment, advice and education. A plan will be provided for patients to seek ongoing wound care via community nursing, GP or self. The wound progress will be monitored at subsquent reviews.
This service is staffed by a Geriatrician, Wound Nurse Consultant, Podiatry and Dietetics.
