Heart Failure Rehabilitation


Northern Health offers a Continence Service for adults requiring assessment and management for bladder and or bowel dysfunction.

Participants involved in the Heart Failure Rehabilitation program will have an individually designed program aiming to reduce their symptoms, optimise function and improve self-management.


What to expect from the service?

The Heart Failure Rehabilitation team includes nursing and allied health professionals. Our team will coordinate and conduct exercise and education sessions with you.

Our rehabilitation program aims to:

  • Keep you out of hospital.
  • Increase your strength and the distance you can walk
  • Improve your quality of your life
  • Increase your confidence and ability to cope with daily activities

Patients who have a confirmed diagnosis of Chronic Heart Failure, and are medically stable, can be referred to the Heart Failure Rehabilitation program. Referrals from all health professionals will be accepted.

Referrals must include:

  • Reason for referral
  • Current and past medical history
  • Information relating to the patientโ€™s condition
  • Current medication(s)
