Support at Home


Northern Health is an approved provider of the Commonwealth Support at Home Program. Support at Home helps you live independently in your own home with a package of care, services and care management to meet your personal needs

The Support at Home Program was introduced in November 2025, replacing the Home Care Packages Program and the Short-Term Restorative Care Program. It improves access to services, products, equipment, and home modifications to help you live independently at home for longer.


What to expect from the service?

We offer a comprehensive range of ongoing Support at Home services and support including:

  • Clinical supports โ€“ such as care management, nursing care, occupational therapy, and podiatry
  • Independence Supports, such as help with showering, taking medications, transport, and respite care
  • Everyday Living Supports, such as cleaning, gardening, shopping and meal preparation.

As well as ongoing care, Northern Health Support at Home have the expertise to support and deliver services under the Support at Homeโ€™s 3 short-term pathways:

  • Restorative Care Pathway: 12-week short-term care to help you regain function and stay independent through accessing coordinated allied health and/or nursing services.
  • Assistive Technology and Home Modifications (AT-HM) scheme: upfront funding to make your home safer and more accessible. It gives you access to products, equipment, and home adjustments to meet your assessed needs.
  • End-of-Life Pathway: provision of additional care in your last 3 months of life. You can be referred for a high-priority assessment for the End-of-Life Pathway, even if youโ€™re not already part of the Support at Home Program. Our team will work in collaboration with your Palliative Care team

We are offering:

  • Qualified and Expert Clinical Care Partners
    Our Clinical Care Partners are all experienced health professionals with specialist aged and community skills. They have the necessary experience, and knowledge to assist with all your ongoing support issues and health needs. Our Clinical Care Partners can help simplify the process of navigating the complex aged and health care system.
  • Consumer Directed Care
    We are passionate about consumer-directed care. We will ensure that a personalised and flexible package of support services is designed with you; specifically based on your identified needs and expressed aspirations. Packages are flexible and can be adjusted as your needs, or your carersโ€™ needs, change over time.
  • Responsive Communication
    Your Clinical Care Partner will contact you regularly. We are a large service consisting of clinical care partners (some of whom are bi-lingual), administrative staff, two dedicated clinical coordinators, a program manager, on-call care management systems and an after-hours support service. Someone will always be available to assist you.
  • Northern Health Connections
    Our connection with Northern Health can assist you to access additional quality health services and interpreters of all nationalities and languages. Staff receive regular professional development training and are experienced in the use of interpreters.
  • Flexibility and Choice
    Our many service contracts give you greater flexibility and choice. You are not restricted to using one agency. Instead, you can select from a number of quality agencies to ensure your specific needs and preferences are met

Inclusion criteria:

  • Age: 65 years or older
  • Indigenous Australians: 50 years or older
  • Homeless/At Risk: 50 years or older and experiencing homelessness or at risk of it.
  • Needs: Difficulty with daily activities (cooking, cleaning, personal care, mobility) due to age or health conditions, needing help to stay independent at home.
  • Residency: An Australian citizen or permanent resident living at home (not in residential care).

Our service targets participants living in the municipalities of Banyule, Darebin, Moreland, Nillumbik and Whittlesea, and parts of Brimbank, Manningham and Moonee Valley.

How to Check Eligibility and Get Started:

  • Contact My Aged Care: Call on 1800 200 422 or visit their website
  • Aged Care Assessment: Undergo an Aged Care Assessment to determine your specific care needs and eligibility for the program.
  • Once assessed by Aged Care Assessment (ACA) Team, approved participants will be centrally recorded with My Aged Care and notified when they have been assigned funding.
  • Once approved, participants can select Northern Health to be their Support at Home provider by directly contacting our service.
