Transition Care Program


The Transition Care Program (TCP) is a short-term program providing care and assistance with recovery for older people who have been in hospital.
The program offers support and low level therapy to allow people to continue their recovery while their appropriate long term care arrangements are finalised.

Transition Care offer two types of services; home-based or bed-based. The home-based program is offered in the patientโ€™s home within the Northern Health catchment area.

To be eligible, a person must:

  • be a current patient in hospital
  • be assessed as eligible by the Aged Care Assessment Service (ACAS)
  • benefit from a time limited, goal oriented coordinated package of care
  • agree to participate in the Transition Care Program, including payment of set fees.
  • be medically stable.

What to expect from the service?

Services Provided:
In consultation with the patient, the treating team will develop a care plan that supports a patientโ€™s care needs and goals whilst on the program.

  • Speech pathology
  • Personal care
  • Home help/shopping assistance
  • In-home respite
  • Geriatrician consultancy
