Care Co-ordination
Care Co-ordination was established in July 2000 at The Northern Hospital.
What does Care Coordination do?
The Care Coordination Service interacts with the Community, The Emergency Department, In-patient Services and Sub-Acute Facilities to ensure effective referral, handover and discharge of patients, and facilitate the most appropriate use of hospital and community resources.
The Continuum of Care
A person's hospital episode is seen as only one component of the care they require. An integrated approach, which recognizes the input of the care providers from the community, the family and the hospital, is necessary for effective care.
For the Emergency and In-patient episodes, Care Coordination can operate as the service, which pulls the components together and ensures that the information sharing necessary for effective discharge planning takes place. |
