What is Advance Care Planning?

Advance Care Planning is a process that may include:
 
*   Discussing your health condition and treatment options with your doctor
*   Talking to your family and friends about your choices for health care
*   Appointing an Enduring Power of Attorney (Medical Treatment)
*   Recording your preferences in a Statement of Choices
 
Enduring Power of Attorney (Medical Treatment)
 
Also referred to as a Medical Enduring Power of Attorney (MEPOA), is a legal document that enables a person to nominate another person to make medical treatment decisions on their behalf.  This person, referred to as the 'agent', can make health care decisions only if the patient is unable to make decisions for themselves.  The MEPOA also has authority to refuse medical treatment on the patients behalf.
 
Statement of Choices
 
A person may wish to record decisions regarding future medical treatments on a Statement of Choices form, to assist the agent to make decisions on your behalf, if you are unable to do so.  While not legally binding, health professionals must take your written Statement of Choices into account when determining medical treatment.
