Geriatric Evaluation and Management Unit - 
Broadmeadows Health Service 

The Geriatric Evaluation and Management Unit provides a high quality aged care service for people experiencing complex health care needs. The primary aim of the service is to assist patient to return to home with an optimal level of health and independence.

To meet patient complex health care needs, individually tailored person-centred care plan are developed and implemented by the specialist multi-disciplinary team, in consultation with the patient, family, and in liaison with community services. 

The major emphasis is person-centred restorative care during the patients stay.

 

 Person-centred goals include restoration of function, nutrition, and skin integrity, managing behavioural problems, and completion of prolonged medical regimens.  There is a high level of turnover, with the discharge of patients to the community after maximum progress has been achieved, provided that adequate care is available.

The aim of the unit is to facilitate the transition of the Graduate Nurse, to that of a confident, competent Registered Nurse through a structured clinically focussed program.  To assist the Graduate Nurse in this transition process, the unit offers specialist nurses provide practical assistance and support, clinical tutorials, and direct supervision in the unit setting. Graduate nurses will also be provided with the opportunity to actively participate in case conferences. 

The Geriatric Evaluation Management Unit is an exciting opportunity for Graduate Nurses interested in working in the area of Gerontology.
