Northern Health Specialist Clinics

All referrals forwarded to Northern Health Clinics are triaged to ensure that clients are referred to the most appropriate service and that referrals are responded to according to priority.

To reduce clinical risk and facilitate access, minimum referral information is required. All referrals must include:

  • Reason for referral
  • Working diagnosis
  • Medical History – current and relevant past
  • Current medications
  • Relevant Investigations and pathology – please include reports and histology
  • Current management of condition and response to this

Please note:  Referrals that do not meet the minimum referral criteria will be returned to the GP with a request for additional information.  The patient will be advised of this.

In addition to the Clinic Statements, Pre-referral Management Guidelines are provided to assist GPs in the management of selected conditions.  These guidelines provide additional specialist clinical information with suggestions for appropriate work up and management prior to referral.
