Northern Alliance Hospital Admission Risk Program (HARP) |
Northern Alliance HARP Program provides services for people with chronic diseases and complex needs.
The multidisciplinary teams that make up the HARP program include specialist Medical, Nursing and Allied Health staff who provide community based care aimed toward preventing an unplanned hospital stay or an emergency department presentation.
Northern Alliance HARP includes representation from Northern Health, Darebin Community Health Service, Plenty Valley Community Health Service, Dianella Community Health Service, RDNS and GP Liaison Unit.
Northern Alliance HARP is located in the Northern suburbs of Melbourne, Victoria, Australia.
HARP Streams of Care:
· People with chronic heart disease
· People with chronic respiratory disease
· People with diabetes
· Older people with complex needs
· People with complex psychosocial needs
The focus of HARP services is to ensure healthcare needs are met to reduce/eliminate avoidable hospital admissions and improve quality of life for participants of a HARP service.
Through a care coordination principle and timely intervention, HARP ensures the essential partnerships between the client, their carer, their General Practitioner (GP), hospital staff and other community organisation are developed and maintained.

We serve those people who are most likely to benefit from integrated care and have the potential to reduce avoidable hospital use through timely, appropriate and culturally sensitive care. This includes people with chronic diseases, with complex needs including people with psychosocial needs. This care is delivered via strong partnerships between primary care, community health, acute/subacute hospital care and other agencies.
