The Northern Alliance HARP streams provide varied services to ensure that all client needs are met. HARP care coordinators ensure that the people we see have best possible care which may see them move between HARP streams should the need arise.

The Northern Alliance HARP streams provide the following services:
Complex need stream
Chronic Disease Stream
Psychosocial needs stream
Interpreters can be provided for those clients who are of Non English speaking background.
HARP employs an Aboriginal and Torres Strait Islander (ATSI) Officer to work with people who identify as ATSI who present at The Northern Hospital ED and to work with the ATSI community, the indigenous community organisations and the HARP services to improve the outcomes for ATSI people with chronic diseases and complex needs. Referring to Northern Alliance HARP
Northern Alliance HARP accepts referrals from hospital inpatient services, hospital outpatient and ambulatory services, community service providers, GP’s, potential clients and their carers. (HARP Psychology is only available to existing HARP patients)

Northern Health Access units will endeavour to refer clients living outside our catchment area to other appropriate services.
