Health Professionals are invited to register for the Advance Care Planning one day workshop.
This full day workshop focuses on the key elements of doing advance care planning including:
ACP in 3-Steps -A Appoint an Agent. C Chat and Communicate. P Put it on Paper.
This workshop is suitable for those health professionals who want to know:
what advance care planning is about
how to introduce the subject to other people
how to assist people with advance care planning decisions
how to complete the advance care planning documents.

Venue: Bundoora Extended Care Centre Conference Room
Note: Registration must be received by 10th October

The pre-reading package must be completed prior to the workshop.

Previously trained RPC Consultants may attend the workshop as a refresher.
